Students & Families,

Participation in athletics in Seattle Public Schools requires registration each school year and a completed and signed physician’s physical every twenty-four months (two calendar years). In addition, this year all students are required to complete a Sport Specific Parent/Guardian Release Form and an Eckstein Eligibility Card (“blue card”).

 

ALL forms must be completed and turned into the Athletics Director prior to participation in practices or competitions.

 

The purpose of the Eckstein Eligibility Card is to help us track student physicals as well as share emergency contact information with coaches. Eligibility cards are available in the school office and all other forms are available for download.

 

Middle school sports offered are:

  • Fall: Girls Soccer, Co-ed Ultimate Frisbee (September – November)
  • Winter: Boys & Girls Basketball (mid-November – mid-February)
  • Spring: Boys Soccer, Co-ed Track & Girls Volleyball (late February – May)

Sign up and coaches contact information will be available 2-4 weeks prior to the start of the season on the Eckstein website. Students should listen to announcements for information about pre-season interest meetings.

Hello 7th Grade Students & Families,

Today students received from their homeroom teacher a permission slip to attend the 7th Grade Field Trip to Lincoln Park and Coleman Pool.  A PDF version of that permission slip can be found below.

The trip will take place on the last day of school, Friday, 06/19.  We hope you can join us for some fun in the sun and swimming.  Buses will depart Eckstein at 9:15 and return in time for students to board their buses home in the afternoon.  If you would like to be a chaperone on the trip, please complete the chaperone section of your student’s permission slip and you will be contacted with details on how you can help.

Students may not participate in the field trip unless the form below has been completed and submitted to their homeroom teacher by this Friday 06/12/09.

We have mailed home all 8th grade sports physicals turned into the main office. Please remember that a physical is good for two calendar years and may be used if the student turns out for a high school sport.
